As Apollo 13 approached Earth for its reentry, the damaged Service Module (SM) was jettisoned from the occupied Command Module (CM) Odyssey. The crew saw for the first time the damage that nearly cost them their lives. Jim Lovell was the mission’s commander (CDR), and adds his momentous radio transmission, “Houston, we’ve had a problem!”, above his autograph.
